Live Well at Iwilei welcomes new applicants year-round. Refer to our checklist to ensure a smooth transition process from health and safety requirements to considering personal care needs. Here are a few key requirements before completing the application:
- 2-Step TB Clearance or Chest X-ray if positive PPD
- Physical Exam within 90 days of admission
- Current Picture ID
- COVID-19 Vaccination Card
